Maintenance Subscription Plan Offer Terms:

  • Subscription plans require a recurring monthly automatic PayPal payment, which will continue until you cancel your subscription.
  • All subscription plans start on the first day of the month following your first payment. (I.e., a subscription purchased on January 15 entitles you to a monthly visit in February.)
  • Monthly subscription plans entitle you to the specified number of appointments per month. If you’re unable to use all of the appointments within the month to which they apply, you may roll over one appointment to the following calendar month. All unused appointments expire at the end of the second month.
  • Appointments within maintenance subscription plans may only be scheduled within normal working hours (Monday through Friday, 10 a.m. to 5 p.m.) except by prior arrangement with Mark Herrera. Unless agreed in advance, each hour of evening, weekend, or emergency service will count as two hours against your monthly allowance of appointment time.
  • You may cancel your subscription plan for a prorated refund within the first 30 days. After the first 30 days of your subscription, all subscription payments are non-refundable.
